This site is a web representation of a UML model conforming to the ISO System of Concepts to support Continuity of Care

The model represented on this site is understood to conform with EN ISO 13940:2016. In addition the model includes concepts in the field of electronic health record communication emerging from the revision and further development of EN ISO 13606.

Each concept is available in RDF using the SKOS terms expressed in XML at https://contsys.org/concept/_concept_name_/rdf.

Known issues

This visualisation of the model re-presents the formal association between concepts in a readable form. The representation is currently incorrectly handling the plural forms of some concept names and the verbs describing the nature of the association between concepts.

Any association classes are incorrectly drawn.

• Bootstrap HTML, CSS, and JS framework for the responsive, mobile first user interface

• Markdown editor is provided using SimpleMDE by SparkSuite
• Client-side GraphViz diagrams generated using viz.js by mdanes. The project builds Graphviz with Emscripten and provides a simple wrapper for using it in the browser.
